👉 Acute Appendicitis with Suspected Perforation
𝑾𝒉𝒚 𝒕𝒉𝒆 𝑺𝒖𝒅𝒅𝒆𝒏 𝑹𝒆𝒍𝒊𝒆𝒇?
A sudden reduction in pain does not always indicate recovery. In acute appendicitis, sudden pain relief may occur when the inflamed appendix perforates, temporarily reducing intraluminal pressure. This apparent improvement can be misleading, as it may be followed by worsening infection, peritonitis, sepsis, and other life-threatening complications.

𝑺𝒊𝒈𝒏𝒔 & 𝑺𝒚𝒎𝒑𝒕𝒐𝒎𝒔 of Acute Appendicitis
• Periumbilical pain migrating to the right iliac fossa
• Right lower quadrant tenderness
• Loss of appetite (anorexia)
• Nausea and/or vomiting
• Low-grade fever
• Guarding and rebound tenderness
• Tachycardia
• Constipation or diarrhea (occasionally)

𝑲𝒆𝒚 𝑾𝒂𝒓𝒏𝒊𝒏𝒈 𝑺𝒊𝒈𝒏𝒔

𝑰𝒏𝒗𝒆𝒔𝒕𝒊𝒈𝒂𝒕𝒊𝒐𝒏𝒔
• Full Blood Count (FBC)
• C-Reactive Protein (CRP)
• Serum Electrolytes, Urea & Creatinine
• Urinalysis
• Pregnancy test (where applicable)
• Abdominal Ultrasound Scan
• CT Scan Abdomen/Pelvis (where available)
• Serum lactate (if sepsis is suspected)

𝑻𝒓𝒆𝒂𝒕𝒎𝒆𝒏𝒕
• Prompt surgical assessment
• Intravenous fluid resuscitation
• Adequate analgesia
• Broad-spectrum intravenous antibiotics
• Monitoring of vital signs
• Appendicectomy (open or laparoscopic)
• Early recognition and management of sepsis where present
