Every year, 102 million units of blood go undelivered across LMIC, not because donors don’t exist, but because the infrastructure doesn’t (Meara JG, et al., 2015). In rural India, the distance between a patient and a blood bank isn’t a gap. It’s a death sentence. One surgeon spent his life refusing to accept that.

Dr. Ravindranath Tongaonkar (1939โ2020) spent 50 years operating in Dondaicha, Maharashtra, where patients were farmers and laborers who would sell their land rather than travel hours to a city hospital. When they needed blood and no bank existed, he championed Unbanked Direct Blood Transfusion: draw from a family member, test rapidly, transfuse directly. Three out of four people in these regions live more than 30 minutes from the nearest blood bank (Kundu S, et al., 2024). For most, UDBT wasn’t a workaround, it was the only option. Then the government took it away.

India banned civilian UDBT in 1998. Dr. Tongaonkar spent the rest of his life fighting the ban that protected non-existent infrastructure. The Blood DESERT Coalition now defines these regions as places where timely transfusion fails in at least 75% of cases, calling for walking blood banks, drone delivery, and autotransfusion (Raykar NP, et al., 2024). He was practicing all of it before anyone had a name for it. The vocabulary finally caught up. The urgency never left.

While the tools are evolving, rural communities need solutions. At TORG, we believe blood access is inseparable from surgical equity, and the pioneers who built solutions from nothing deserve to be remembered, studied, and followed.
