𝗔𝘀𝗲𝗽𝘀𝗶𝘀 𝗶𝗻 𝘁𝗵𝗲 𝗢𝗽𝗲𝗿𝗮𝘁𝗶𝗻𝗴 𝗥𝗼𝗼𝗺: 𝗔 𝗚𝗹𝗼𝗯𝗮𝗹 𝗜𝗺𝗽𝗲𝗿𝗮𝘁𝗶𝘃𝗲 𝗳𝗼𝗿 𝗦𝗮𝗳𝗲𝗿 𝗦𝘂𝗿𝗴𝗲𝗿𝘆

Surgical Site Infections (SSIs) remain one of the most persistent challenges in global surgery, undermining patient safety and straining healthcare systems worldwide. According to the 𝑾𝒐𝒓𝒍𝒅 𝑯𝒆𝒂𝒍𝒕𝒉 𝑶𝒓𝒈𝒂𝒏𝒊𝒛𝒂𝒕𝒊𝒐𝒏 (𝑾𝑯𝑶, 2016), SSIs threaten the lives of millions of patients each year and significantly contribute to the global burden of antibiotic resistance.

In 𝒍𝒐𝒘- 𝒂𝒏𝒅 𝒎𝒊𝒅𝒅𝒍𝒆-𝒊𝒏𝒄𝒐𝒎𝒆 𝒄𝒐𝒖𝒏𝒕𝒓𝒊𝒆𝒔 (𝑳𝑴𝑰𝑪𝒔), approximately 𝟏𝟏% 𝒐𝒇 𝒑𝒂𝒕𝒊𝒆𝒏𝒕𝒔 𝒖𝒏𝒅𝒆𝒓𝒈𝒐𝒊𝒏𝒈 𝒔𝒖𝒓𝒈𝒆𝒓𝒚 𝒅𝒆𝒗𝒆𝒍𝒐𝒑 𝒂𝒏 𝒊𝒏𝒇𝒆𝒄𝒕𝒊𝒐𝒏, while in 𝑨𝒇𝒓𝒊𝒄𝒂, as many as 𝟏 𝒊𝒏 𝟓 𝒘𝒐𝒎𝒆𝒏 (𝟐𝟎%) who undergo 𝒄𝒂𝒆𝒔𝒂𝒓𝒆𝒂𝒏 𝒔𝒆𝒄𝒕𝒊𝒐𝒏𝒔 suffer wound infections. These infections not only endanger mothers but also affect neonatal survival and maternal capacity to provide care.

Yet, surgical site infections are not confined to resource-limited settings. In the 𝑼𝒏𝒊𝒕𝒆𝒅 𝑺𝒕𝒂𝒕𝒆𝒔, SSIs account for over 𝟒𝟎𝟎,𝟎𝟎𝟎 𝒂𝒅𝒅𝒊𝒕𝒊𝒐𝒏𝒂𝒍 𝒉𝒐𝒔𝒑𝒊𝒕𝒂𝒍 𝒅𝒂𝒚𝒔 𝒂𝒏𝒏𝒖𝒂𝒍𝒍𝒚, incurring an estimated 𝑼𝑺$ 𝟗𝟎𝟎 𝒎𝒊𝒍𝒍𝒊𝒐𝒏 in excess healthcare costs each year (𝑾𝑯𝑶, 2016).

𝗚𝗹𝗼𝗯𝗮𝗹 𝗖𝗼𝗺𝗽𝗮𝗿𝗶𝘀𝗼𝗻 𝗮𝗻𝗱 𝗜𝗺𝗽𝗹𝗶𝗰𝗮𝘁𝗶𝗼𝗻𝘀

The disparity becomes even more striking when viewed through the lens of international incidence rates. A study by 𝑭𝒂𝒏 𝒆𝒕 𝒂𝒍. (2014) demonstrated wide variations in SSI incidence across different countries.

Nations such as Ethiopia (≈21%), Tanzania (≈20%), Central African Republic (≈18%), and Iran (≈15%) recorded 𝒕𝒉𝒆 𝒉𝒊𝒈𝒉𝒆𝒔𝒕 𝑺𝑺𝑰 𝒓𝒂𝒕𝒆𝒔 globally.

In contrast, countries like Germany (≈2%), France (≈2%), Malta (≈1.8%), and the USA (≈2.5%) reported 𝒕𝒉𝒆 𝒍𝒐𝒘𝒆𝒔𝒕 𝑺𝑺𝑰 𝒊𝒏𝒄𝒊𝒅𝒆𝒏𝒄𝒆𝒔.

This disparity illustrates the enormous gap in surgical safety and infection control standards worldwide. If the 𝑼𝒏𝒊𝒕𝒆𝒅 𝑺𝒕𝒂𝒕𝒆𝒔, with its low SSI rate, spends nearly 𝑼𝑺$ 𝟗𝟎𝟎 𝒎𝒊𝒍𝒍𝒊𝒐𝒏 𝒂𝒏𝒏𝒖𝒂𝒍𝒍𝒚 managing these infections, the 𝒆𝒄𝒐𝒏𝒐𝒎𝒊𝒄 𝒂𝒏𝒅 𝒉𝒖𝒎𝒂𝒏 𝒄𝒐𝒔𝒕 in countries where SSIs exceed 𝟏𝟓–𝟐𝟎% is likely 𝒔𝒕𝒂𝒈𝒈𝒆𝒓𝒊𝒏𝒈𝒍𝒚 𝒉𝒊𝒈𝒉𝒆𝒓 — especially when considering the limited healthcare budgets and inadequate infection-control infrastructure in many LMICs.

For patients in these settings, untreated or poorly managed SSIs often lead to 𝒄𝒉𝒓𝒐𝒏𝒊𝒄 𝒘𝒐𝒖𝒏𝒅𝒔, 𝒔𝒆𝒑𝒔𝒊𝒔, 𝒑𝒓𝒐𝒍𝒐𝒏𝒈𝒆𝒅 𝒅𝒊𝒔𝒂𝒃𝒊𝒍𝒊𝒕𝒚, 𝒂𝒏𝒅 𝒆𝒗𝒆𝒏 𝒅𝒆𝒂𝒕𝒉. Beyond the individual, these infections amplify the burden of 𝒂𝒏𝒕𝒊𝒎𝒊𝒄𝒓𝒐𝒃𝒊𝒂𝒍 𝒓𝒆𝒔𝒊𝒔𝒕𝒂𝒏𝒄𝒆, increasing healthcare costs and overwhelming already strained surgical services.

𝗧𝗵𝗲 𝗪𝗛𝗢 𝗚𝘂𝗶𝗱𝗲𝗹𝗶𝗻𝗲𝘀: 𝗔 𝗖𝗮𝗹𝗹 𝘁𝗼 𝗥𝗲𝗶𝗻𝗳𝗼𝗿𝗰𝗲 𝗔𝘀𝗲𝗽𝘁𝗶𝗰 𝗣𝗿𝗮𝗰𝘁𝗶𝗰𝗲

To curb this global challenge, the 𝑾𝑯𝑶 𝑮𝒍𝒐𝒃𝒂𝒍 𝑮𝒖𝒊𝒅𝒆𝒍𝒊𝒏𝒆𝒔 𝒇𝒐𝒓 𝒕𝒉𝒆 𝑷𝒓𝒆𝒗𝒆𝒏𝒕𝒊𝒐𝒏 𝒐𝒇 𝑺𝒖𝒓𝒈𝒊𝒄𝒂𝒍 𝑺𝒊𝒕𝒆 𝑰𝒏𝒇𝒆𝒄𝒕𝒊𝒐𝒏 (2016) provide an evidence-based framework to strengthen asepsis in operating rooms.

Key recommendations include:

Strict 𝒉𝒂𝒏𝒅 𝒉𝒚𝒈𝒊𝒆𝒏𝒆 before and after patient contact

Use of 𝒔𝒕𝒆𝒓𝒊𝒍𝒆 𝒔𝒖𝒓𝒈𝒊𝒄𝒂𝒍 𝒂𝒕𝒕𝒊𝒓𝒆 and appropriate barrier precautions

Ensuring 𝒔𝒕𝒆𝒓𝒊𝒍𝒆 𝒑𝒓𝒐𝒄𝒆𝒔𝒔𝒊𝒏𝒈 𝒐𝒇 𝒔𝒖𝒓𝒈𝒊𝒄𝒂𝒍 𝒊𝒏𝒔𝒕𝒓𝒖𝒎𝒆𝒏𝒕𝒔

Proper 𝒆𝒏𝒗𝒊𝒓𝒐𝒏𝒎𝒆𝒏𝒕𝒂𝒍 𝒄𝒍𝒆𝒂𝒏𝒊𝒏𝒈 𝒂𝒏𝒅 𝒂𝒊𝒓 𝒄𝒐𝒏𝒕𝒓𝒐𝒍 in operating theatres

Rational 𝒂𝒏𝒕𝒊𝒃𝒊𝒐𝒕𝒊𝒄 𝒑𝒓𝒐𝒑𝒉𝒚𝒍𝒂𝒙𝒊𝒔 guided by evidence-based timing and indication

Adopting these practices consistently can 𝒓𝒆𝒅𝒖𝒄𝒆 𝑺𝑺𝑰 𝒓𝒂𝒕𝒆𝒔 𝒃𝒚 𝒖𝒑 𝒕𝒐 60%, even in resource-limited settings.

Maintaining asepsis in the operating room is not a local protocol — it is a 𝒈𝒍𝒐𝒃𝒂𝒍 𝒓𝒆𝒔𝒑𝒐𝒏𝒔𝒊𝒃𝒊𝒍𝒊𝒕𝒚. As surgical capacity expands across the world, so must the commitment to infection prevention. The 𝒆𝒄𝒐𝒏𝒐𝒎𝒊𝒄 𝒕𝒐𝒍𝒍 seen in high-income nations like the United States highlights how much more devastating the impact can be where healthcare systems are fragile and infections are preventable but untreated.
